I almost never use my middle name. I use it when required.

Only when required on something legal. I mean really required, even my drivers license has my maiden name instead of my middle name. No one in my family says my middle name, unless we are having a discussion about names for some reason.

Never. I've got two of them and they just never come up. MY first name is enough of a mouthful to throw in a middle one or two LOL!

If my mom used my middle name, it meant that I was in deep doo-doo, so I don't use it at all unless absolutely required.
