It could mean they have had damage to their throat or vocal chords. My husband has had a problems with warty growths on his vocal chords for almost 30 years. They would grow and keep his chords from vibrating properly so his voice would be a little hoarse. Then he started having operations to remove them, each time a different technique, supposedly so they wouldn't grow back. They always did. 5 times this happened. The last time me almost died during the operation twice, so we said enough! So now he lives with growths and a lot of scar tissue making his voice even worse:-(

Happened to me. I went to an ENT doctor who looked down my throat and saw the problem. I had a smoke allergy and took an allergy medication to get my vocal cords back on track. It is a very serious issue, and the vocal cords can be damaged... so it is very important to not strain them.

My brother got that way and got diagnosed as having Emphysema. He smokes.

My ophthalmologist had this problem.Turned out to be acid reflux disease that was damaging his vocal cords.

1. Is he a public speaker? Talking too loudly can make you hoarse. 2. Does he drink/smoke a lot? this damages the larynx. 3. Does he eat a lot? especially before bed, this causes acid reflux. 4. Does he cough a lot? this can tense the chest making talking more difficult 5. Does he eat a lot of dairy? this can produce phlegm.
