Yes, your cell phone is constantly being monitored by what is called the HLR or Home Location Register and the VLR or Visitor Location Register (used when roaming). This has to happen to route calls efficiently. If the network didn't track your cell phone constantly you could not receive any incoming calls. Outgoing calls placed from your cell would require 2 calls, the first would always be to your service providers network to request your radio time and let it know that you are powered up and ready to transmit and receive signals.

It can pin point you to a specific tower. Each tower covers an area which is dependent upon the surroundings. In a large city, for example, this area will be very small but in the clear, open country it will be quite large (15 square miles). Also, if you are within the range of 2 or more towers it can limit the area even further!
