Well sometimes as Auntie Em says, they may genuinly believe the person could do with some form of psychological help. SOmetimes people express objectionable or offensive thoughts to others around them as a cry for attention because they secretly know that they do need help, and feel they need someone else to guide the way. Sometimes its because its easier to dismiss someone as being "disturbed" or "crazy" than actually deal with what they're saying. If a guy's crazy, then you don't have to listen to him, right? If they literally say "go get help" rather than explain compassionately that they think you need some help, this might be your answer. It does depend a lot on the situation though. If you're asking about something specific that's happened to you, we probably need more info.

It's a well-known trick coined by the Christians. By doing that, they feel better about themselves, superior while, at the same time, giving the appearance of being caring, loving and helpful

"Of course you're crazy otherwise you would agree with me on everything." What else could it mean? They are brilliant and perfect so you must be the one whose head is screwed on backward. My wife has taught me the perfect response for those situations. "You may be right. Bye"

It's sarcasm...usually. But, for some, I'm sure it's meant to be taken literally.

Because they have an opinion that they consider normal and when you encroach it they consider you mad.
-
Because they think saying it makes them sound cool, and as George Carlin said,"Deep down, everyone just wants to be a cool guy"
-
Your opinion here is valid but not a given. There are plenty of people who really do need to go get help and that answer does suit them.

It's another form of insult, implying that the one who managed to upset the whiner- that the whinee is somehow mentally deranged and in need of psychiatric help.
