No. If I put it on my lap, it usually falls on the floor. I use it to wipe the food off my mouth and having it on the table makes it more accessible.

At a table-service restaurant I do. At home or at a casual restaurant, I do not.

Yes, but my dad usually makes me wear a paper napkin tucked in my shirt collar so I don't get any food stains on my shirt. My dad has a two napkin rule for us kids. (One tucked in shirt collar and one on the lap)
