I always thought of it as a shelf. Some doctor thought about this, I guess. My Army barracks in Berlin, Germany, had toilets like these and they had the tiniest drains for toilets I have ever seen.

There is not a stool. It is just that the inside of the bowl of the toilet is shaped a little differently than American Toilets. Instead of your poop dropping directly into the flat bottomed bowl like in America (and maybe splashing you), there is a raisd bottom, the poop lands here and when you flush, the water pushes it off into a lower hole and into the sewer (it is not as strange as it sounds). The reasons for this design are unknown to me, I have heard many different ones, so who knows. Better than the hole the ground toilets I have had to use in Italy and in the middle east where you just squat over a hole in the floor. I hate those.
