It means crazy, or insane.....not all there. The light are on but nobody's home, a few sandwiches short of a picnic....these are all just witty ways to say someone is nuts.
A person is totally sane, with adequate knowledge to be accepted and revered.
Why should I pay attention? What has attention ever done that it deserves to be paid?
