He/she isn't required to do anything but the majority of football QB's will say set and will lift their left leg up so that if the center did not hear him call "set", he will see that the QB lifted his leg and will be ready to hike the ball.

Normally the quarterback will lift his leg in the air to where the center can see it, if the verbal count or snap was not heard. I do not know this for fact, but I believe it is a silent count in shotgun formations.
