The development of a malignant neoplasm (a cancer) is a multistep process from which normal cells become abnormal cells that proliferate. The risk factors you mentioned increase the risk of cancer.

There are certain items which are known to cause cancer, while other items are known to increase the 'risk' of cancer. Cancer is a diverse class of diseases, each of which differs widely in their causes and biology. The common thread between all cancers is the acquisition of abnormalities in the genetic material of the cancer cell and its progeny. if we figured out this miracle then we would not be losing so much money to the search!

The transformation of a normal cell to a cancer cell occurs with one or more spontaneous mutations to the DNA. This can be caused by radiation, chemicals, or viruses. Nobody knows (of course!) the exact mechanisms. Cells that undergo a higher rate of mitosis seem more vulnerable, which is why so many cancers arise from epithelial linings of organs (breast, lung, colon, skin, etc.) where there is a high cell turnover rate. A cancer derived from epithelium is called a carcinoma.
