What happens to credit card bills when the credit card holder dies? Is the estate responsible for paying the bill off, the spouse, the children? Would the family have to use the life insurance proceeds to pay it off?

No, the family doesn't have to pay anything. It was the credit card holder's debt, not theirs. The credit card companies will probably try and get money from anyone willing to pay, but it's not the family's responsibility. The life insurance is for the family to take care of themselves, not to pay any debts incurred by the deceased. If there is an estate, it is possible that it may come out of that, however I'm not sure about that part.
