    I'd say yes. Quite often wars are last-minute emergencies, with not enough time to adequately train all troops sufficiently, and give them lots of practice with their weapons. Mistakes will happen.

    Yes. By definition, the unexpected will happen in war, because the enemy is making it so. And people (and equipment) do not always handle the unexpected right.
