A doctor needs to know alot of information in order to do his/her job properly. As long as the question is relelvent then it would be silly to get offended by it.
All they need to ask you , if they do at all, is ask you if you are sexually active?

That's almost exactly how I phrase it. It's direct and to the point, without any euphemisms. Nobody's complained to me about the question.

rephrase: this is nothing personal,just for a patient history....are you sexually active with the opposite sex,...do you engage in sexual intercourses with the same sex?.....if the doc would say it the right way or somehow professional,then i wouldnt be offended.
and he/she better ask it quickly and direct to the point so that it wouldnt sound too personal.
and he/she better watch out with his/her facial expression and tone when asking.
