There is a way to seemingly do this. It is a method called cyclical breathing and many wind instrument players can to this. They continue to blow out of their mouths to sustain a note while they breathe in through their noses but this is done by blocking off the back of the mouth with the toungue while inspiration is taking place through the nose, just look at dizzy gillespies cheeks and you'll better understand how this happens. Other than this it is impossible to breathe in while we are exhaling and visa versa.

  • You can't. When some says breathe in with your nose and out with your mouth they mean first inhale through your nose then exhale through your mouth.

Just because that's how are bodies are designed. Just like you can't open and close your eyes at the same time. If you want to know why you will have to ask whomever or whatever designed the human body.
