Is it required to fill out an incident report form when an employee gets hurt on the job?

WC requires a "supervisors first Report of Injury" form if a claim is filed. I always did that form even though I may not think the injured was going to seek medical attention or lose time. It was a good record of incidents and CYA. Besides my company required it. It may be law, for any company who's employee's are covered by WC.
