It depends on what the cause of the pain is. I'm sure breaking your neck or getting shot hurts like nothing else imaginable, but mental things like depression, heartache and hallucinations can be pretty bad, too. From personal experience, being bothered by OCD sucks pretty horibly bad, so does heartache. Although, I've never really had heartache that bad. The worst physical pain for me was probably smashing my chin on the side of a swimming pool, or having bone or something taken out of my back. I don't really remember either one of those.
