The testicles are removed, the glans is shaped into a clitoris, and the scrotum and "shaft" are used to make the vagina. There are websites that explain this in detail if you're so inclined.

Yep, Rince is correct. It is cut and manipulated to create the vagina. They use it so that there is still the sexual sensation. You really don't want to see that surgery...it's very uncomfortable to watch. I can't even imagine actually going through it.
