If he doesn't intend on hurting anyone then keep him away from any mental institution. Find a great caring therapist. The right one can work wonders, allmost miracles. If he's not having hallucinations then avoid neuroleptics. The right drug cocktail can work wonders too. A good psychologist and a good psychiatrist. Find two that work together. Get personal recommendations if you can.

You don't say much about the age of the person, or if male or female, or if drugs brought on this condition, or other situation. You say oldest friend, does this also indicate the person is older? Schizophrenia starts early, mostly teenage years and progresses as the person ages. Was this person ill from a younger age? If not, he/she may have been misdiagnosed as well. It could be a recent situation that may have thrown this person into some stage. None the less, be as it may, a good breath therapy, deep rolfing therapy and other such somatic activation, as well as cognitive/rational emotive therapy, may help this person to activate their instincts and emotions and clear their thinking which is disassociated. Keeping eye contact and physical contact is important, as they are too disconnected from the world and go into their own, which is fantacy orientated, but take it easy as the person needs to learn to trust.

Listen and don't judge him, my x-husband is schizo and bi-polar along with personality disorder. He was originally just bi-polar but messing with drugs and his meds he developed the others. He should see a therapist and keep it up, also I am sure they will add medicine as well. It is important to stay focused and on task, if he is feeling down he needs to see his therapist and be honest with how he is feeling and what is going on, if he isn't honest then he will not get the help he needs. Just try to understand and maybe do some reading on the disorder. Good luck!

If he asks for help from you, you could provide it. However, I suspect he just wants a friend. Act just like you always have, and treat him like a normal person. If you have any thoughts about suicide or think someone else does, consult the suicide prevention sites online. You will see the the decision has nothing to do with the actions or non-actions of other people.
