As this is in the section on the Christian Bible, let me give an answer based on the Bible. Jesus said,
13Enter ye in at the strait gate: for wide is the gate, and broad is the way, that leadeth to destruction, and many there be which go in thereat:
14 Because strait is the gate, and narrow is the way, which leadeth unto life, and few there be that find it. (Matthew 7:13-14)
So, according to Jesus the way to heaven is very narrow. There are not a whole bunch of paths leading there. There is but one. It is not an easy path to find and stay on. Few actually do, but find it we must if we are to go to heaven. Which path is correct is what the debate between the various sects of Christianity is all about.

According to Jesus, there's only one way: through him. Meaning, you have to follow him to get to Heaven. Of course, what following Jesus means is different to many people--Christianity is an extremely diversified religion!
Different religions have different answers to this question. Some religions don't even believe there is an afterlife, let alone a heaven/hell dichotomy.

That depends on your concept of heaven, as there are many, many believe that you have to die to make it to heaven, hey sux to be you, yet others believe they can catch a flight to some tropical heaven like in those Corona commercials, but I myself know a short cut to heaven and I go there quite often, though im not going to tell you how to get to my girlfriends house, I suggest you go get your own slice of heaven.

Evenutually all souls will return to God. The bible speaks of Universal Salvation, God wants that no soul should perish. Also, if God resides in you and you and him, how can you perish and he remain intact. Evenutually all souls return to God. If you believe in a creator, it's the only plausible explanation.
