Remove the components of the dash below or around the lighter. This might be by reaching behind plastic fittings to make the center pin of the holder pop up, allowing the fitting to be removed, then the plastic. Or you might need to use a philips head screwdriver and disassemble the necessary parts to get to your dash. Then the cigarette lighter holder can be removed from behind. Some of the Toyotas will have a slide that pins the whole thing to the dash. The lighter itself has a two wire connector that pulls straight off the lighter.
Finding a replacement is tough. Check with your dealer. On a truck, we had ours fail -- and it was some kind of small jumper wire for the center post (+) that died. These are crimped on rather than soldered -- maybe because of the heat?
