What is the process for getting somebody's name in the prayer book, so the people at the temple will pray for them? would the requester have to know (or find out) whether or not the person they want a prayer said for is mormon?

Actually it is really easy. You just call one of the temples (numbers in the phone book) and ask that they put the name on the prayer book. The person calling does not have to be LDS, nor does the person whose name is being submitted have to be LDS. Alternatively, if you are LDS there are special tables where you can write down the name to be put on the prayer book.
