File a name change form and the real father might have to take a test to prove how his is to be put on the B/C. Call vital records in your state they can help you.

The biological father needs to sign an affadavit to acknowledge paternity (or a DNA test can be performed, but the first way is much less costly), then the two of you can file a petition for a name change of a minor. As long as both biological parents agree to the change, it will be granted. It costs about $40.
