If the pitch hits the ground before the batter contacts it, is it a fair hit?

    Yes, the only time a pitch is dead is when the umpire calls time before the pitcher throws or the pitch hits the batter, you can hit a pitch even is the pitcher balks, it is a "free swing" if you get out you get stay and finish your at bat, it is live if you hit safely.
