The infield describes the area of field (in fair territory) that includes everything that is not the outfield. So yes it applies to the pitcher and catcher too.

The infield fly rule is invoked whenever a fly ball is easily catchable by an infielder (given appropriate baserunner/out situations and a fair ball). Therefore even if an infielder backs up into the "outfield" an infield fly can still be called. And once the pitch is released the pitcher is considered an infielder.
