It does not include the driver's tip. The driver's tip (assuming the order gets to your house in a timely manner) is usually 15% (or more) of the total bill, like for any other service.

I think it probably depends, and you ought to ask someone from that particular establishment. I've ordered from places where the "delivery charge" went to the delivery person and from other places where the "delivery charge" goes to the house. Best to make sure.

I really think that it is an extra fee. Giving a tip to the driver is something extra out of our hearts.

That pisses me off because the pizza is already expensive to begin with.

The tip is extra. The fee is to cover the cost of fuel. Remember that in the US employees aren't paid very much (unless state law mandates it) because they earn tips, your tip might constitute almost all his/her salary.

I know when I used to work for a pizza place we had an extra fee for deliveries outside of our normal area and the drivers didn't see any of it which really sucked because people didn't want to tip because of it and because it was so far they would miss out on other deliveries...
