    Plans

    The plans are Signature and Classic and may vary by state. The Signature plan pays no deductible and no copay. The Classic plan requires that the patient pay a $3 or $5 copay.

    Eligibility

    To be eligible for this program, you must be eligible for Medicare Part A or enrolled in Medicare Part B and live in the service area of the plan.

    Pharmacy List

    The pharmacy list tells you which pharmacies are participating in the prescription drug plan.

    Drug List
