A hiatal hernia occurs when part of the stomach pushes up into the esophagus. Most hiatal hernias are small, and often present no symptoms. However, some can grow quite large and require medical intervention; in rare cases, doctors have found that the entire stomach has pushed itself into the esophagus and chest cavity.
