I believe that you use Dr. John Doe in a familiar way. When you write, you should use John Doe, M.D.. That is the way it is listed in phone books and medical journals. I have not seen them both used in medical journals or other publications.

Yep, use Dr. and M.D. in the same title - as someone can be Dr. John Doe because they have a PhD in Theology (for example). M.D. specifies they are a medical doctor... The woman who took my Spin class (at the gym) did a PhD in English Lit or something similar. She was very proud to be a Dr. - I called her the Spin Doctor, much to her amusement...

In India we use Dr. before the name of physicians, but not his qualification like MD or MS behind his name. We feel it is not necessary to add his qualification to his name.
