Yes, the parking lot.

Yes. My freshmen year in high school 1989. The school had a indoor smoking area for the students. It was closed my sophmore year because the state passed and law saying no smoking inside any gov. building. So the teachers could not smoke inside either. This was in Illinois, they have since passed laws prohibiting smoking in any public building. Even private business owners cannot smoke in their place of business.

No, smoking wasn't allowed.

No. But, in the official student handbook, it just said that smoking wasn't allowed. So, when I was 18, and could legally possess smokes, I walked around like Baretta with an unlit cigarette dangling from my lip. I quit smoking many, many years ago.

Yep... There was an outdoor courtyard between the cafeteria, The Corral, and the Pool. I think they stopped that a couple of years after I was there - 75 or so. But then, kids had permission to go out of the school for lunch, so they'd probably smoke quite a bit, then. I also remember that when I was there, they'd even taken the doors off the bathrooms, because kids would go in there and smoke.
