Is it illegal to audio-record (voice recorder, etc.) a doctor visit without their knowledge?

It depends on your state, but for most of them the answer is yes. It must be you doing the recording however...you may not record a doctors visit between two other people without notifying EITHER of them. http://en.wikipedia.org/wiki/Telephone_recording_laws
