A physical assessment of the child and then the parents - if they are too distraught to make any sense, then the grandmother. Unless the grandmother was there, and the parents were not, when the accident/illness ocurred, in which case she would need to explain what happened.

i would ask his parents to clam down maybe get some water. ask the grandma whats going on and the kid if he is able. but im not a nurse, so who knows?

The child is the most important person. You need to visually assess the child and then need to ask where the hurt is as # 1 and then what type. You then assess the child physically for signs that relate to the type and site of the injury. Calming the parents and getting answers to your questions can be included in your assessment of the child.

Emergency means you start with the child's body and heal it as much as possible. The rest can wait for another observer. Later with what you are observing and present with staunching the wounds or whatever is needed.
