It's been a while since the last time I had an English exam! I used to first memorize the new vocabularies and then study the grammar. Websters is a good dictionary to learn the American pronunciations and Oxford is the best choice if you want to learn how to use words in sentences correctly.

It depends on the topic. I've never seen an exam in this subject that was broad enough to cover it all. So it depends on whether you're studying Shakespeare, Romantic Poets, Existentialism, Transcendentalism ... or spelling, grammar and syntax. I'd study what I expected to be on the exam.
