Erm, depends what you are going to the doctor's for. If you have a headache and your doctor asks you to get naked...i'd get another doctor!

Depends why you are going to see the Dr. He will only have you remove clothing so that he can examine a specific spot. If your foot hurts, no you will not need to remove your clothes, but yes your shoes. If he wants to listen to your heart/lungs then he will reach up into your shirt. Drs. see naked people 100s of times a week. For them it becomes common place. Nothing to worry about.

If you are getting a physical, it depends on how thorough it needs to be. He may just check your blood pressure, listen to your heart, look at your mouth, eyes, etc. He won't necessarily take your clothes off unless whatever you are getting the physical for requires a thorough physical. If you are just going to examine an ailment, you won't to take your clothes or shoes off unless the problem lies beneath them. Even if you have, say, a rash on your stomach, he probably won't make you take your shirt off, just make you lift it up.
