Insomnia is a little of both. Some people have high anxiety that may result in acute insomnia. Others (me) have chronic insomnia that stems from a time in their life when they had very high anxiety, and the insomnia just stayed with them. Insomnia also comes hand in hand with depression.

It can be both, as Talim said: if you are anxious or stressed, this will lead to an inability to sleep. But it could have physiological origins too...anyway, there is a fine line between the body and the spirit...what affects one usually affects the other.

Isn't "all in your head" the very definition of a psychological problem?

A little of both. More or less depending on the type. Mine is more physiological because I have CRD. I like to say "My biological clock is broken".
