Why do people assume your cloths with be dirtier if you dont wear underwear? Arnt most modern people clean enough down there that it shouldnt matter if you wear underwear or not? If thats not the case then isnt underwear serving as diapers?

It is not about being clean, it is about leaking. We all leak a bit, and an important function of underwear is to soak up the leakage. That is also why you need to change your underwear occasionally even though all it has ever been in contact with is you (clean) and the inside of your clothes (also clean). And yes, the underwear does serve as 'diapers', for the very small amount of leaking we usually do. If it is more, then more effective diapers are needed, such as the sanitary napkins girls use during menses or the bulky grownup diapers old people use when they have lost control of their bladder or spincter.
