I've accessed all 7 terminals in Halo 3, read them completely on single player campaign on Heroic. Why won't the achievement unlock?

You must have died there somewhere and your progress as far as it was concerned did not include one or more of the terminals.

I have gone through it several times now. I did it once playing the through the entire three levels that include terminals and it did not give me the achievement. Then I looked it up and found out that it had to be on single-player so I did that. Again I played through the entire level then saved and jumped to the next level that included terminals. Then I still didn't get it so I watched a Youtube video that showed that you do not have to play the entire level as long as you save and quit after finishing the terminals in each level. I went back and did it again without running through the entire levels and I still haven't gotten the achievement. Nowhere have I read that you can't die while getting this achievement and that wasn't the case when I got the achievement on an old profile.
