ITS ways to get your teeth fixed its not a big deal
If they have buck teeth already and they suck their thumb so what. It simply means that their mouth has more room for their thumb and is, therefore, more comfortable for their habit. If they're bothered by having buck teeth they can find someone who either doesn't care about their occlussion (someone who can look passed that is probably a better person anyway) or someone who prefers that, finding it attractive (as many do, there is an internet site focus on "bucktooth lovers").
It work to some avantage

The thumb or the head?
Either would work. The head would be more effective, as you would only have to amputate one appendage to stop the action, as if you just chop off one thumb, there's another you may have to worry about.
