'No sex' would mean total abstinence from sex. This is something many people would find difficult to practice. So the question is - what you are trying to be safe from. From sexually transmitted deceses or unwanted pregnancy? Or from emotional involvement? All these can be overcome to a considerable extent by sticking to a single partner in sex for life. Then safe sex need not mean 'no sex'.

Yes, unless it's within a marriage where you know you'll only be with that person. The only thing that prevents pregnancy and STDs 100 percent is abstinnence. However, some items and some techniques can reduce the risk when you are doing it.
