How do I split power from a single device to 2 different voltages? I require a 5 & 9 volt input and have 9, 12, & 15 volt power supplies available.

You can't, at least feasibly. Your best bet is to go to Radio Shack and buy the proper power supply's. It will save you a lot of headache.
