They may have had cosmetic surgery to shave the jaw, lessen the Adams apple, and laser away face and body hair etc. They are on the female hormones and may have had breast implants. They have yet to go all the way having the phallus sniped and a vaginal cavity created. The lesser shemales are just that, females going to male. The change is usually not as drastic. Most is done via hormone treatments. Most women going to men don't have fake Adams apple installed or get hair plugs on the face since hormones will do that. About the biggest major thing they will do is have the breast removed and a phallus built.
