I don't have much background in this but this is what I know. I hope it helps. They can perform them in house using public databases or they can purchase software that aggregates public information and use specialized search engines to search for US and international patents. They also use outside search firms that specialize in Patent searches. Either way they (Patent attorney's office/searches) may be very familiar with the databases and so you are essentially paying for their experience and technology.
