You will need to learn at least Illustrator and InDesign, alongside Photoshop. Dreamweaver and Flash are also a big help. Then you will need to get a portfolio together of your best work. Ask friends and family if they have anything they need designing - maybe there is a family company that needs a new logo. Do a few jobs like this for free in order to build up your portfolio. Record the steps you take in your design process - research, sketches, different routes, etc. Clients love to see this. Do some designs for places like threadless.com You will also need an online portfolio - you can use a blog site such as wordpress, but it really is better to design your own bespoke website. Show off those skills! Create flyers to promote yourself and get them printed. Send them to small and medium sized local businesses. Prepare for a lot of rejection, especially at first. Don't be picky about what you do!
