Possible causes: Thermostat bad, vacuum leak, leak from water pump or hoses, faulty radiator cap, blown head gasket, automatic transmission problems leading to overheating via transmission cooler in bottom of radiator, clogged radiator/block/head passages from use of plain water rather than 50/50 coolant mixture.

you definately have a leak in the cooling system. i would have to disagree with vacuum hoses or trans problem. your coolant is going somewhere. find the leak, fix the problem. if you dont see coolant on the ground then you could have a blown head gasket which means the coolant is leaking into the engine and being burned and sent out as steam in the exhaust. does there seem to be more smoke behind you than normal?

Could be the clutch in the fan is bad.
