Probably from some injury that stopped blood flow to the tail. I had a kitten once that got in the way when I was cleaning the oven, some oven cleaner dripped onto the last inch of his tail. It fell off, ick. In that case the acid in the oven cleaner ate through the tip of his tail.

My cat got a dead tail because my little sister decided it would be a good idea to put a rubber band around it, at about half way up. She said something to me when I came back the next day, that there was something wrong with Kitty's tail. SHe told me she made the kitty a "pony tail".
