Generally on the steering wheel.

Wherever they need to be to control the vehicle. I drive many different style vehicles and the controls are different for each one.

Usually at 3 & 9 on the steering wheel. When I'm driving through town I'll usually have one hand at 9 and the other on the shifter.

My left arm rests on the window sill, with the hand on the wheel at about 9. The right hand usually sits in my lap.
