I think it an anal douche for washing out his rectum. Without wishing to put stuff in your head, gay men use them to clean themselves out before anal sex. Im gay and i use one, most of us do...its different to the one you are describing but yours like an older one. Not that im saying your boyfriend is gay...he might just be erm...very clean.
