It does need to be notarized and the number of witnessess that are required varies from state to state.

There are many different types of a POA's (e.g. durable, non-durable, special, etc.). Some of them have special statutory requirements to form in some states, but most do not. Thus, you can usually create a POA with just a handwritten scribble saying I give X power of attorney. You do need to have the POA notarized, but the notary is usually the only witness that you need.

You don't need a lawyer, but it must be witnessed and notarized to be valid. You can find forms on the internet, or can purchase them from places like Office Max/Depot.
