Female cats in heat can certainly act like they are in pain. However, this is normal, and the "pain" is simply the hormonal drive to breed. It sort of a severe case of horniness, which can certainly be uncomfortable, as most humans know. The male cat has bristly little hairs on his penis that point in a direction that causes a somewhat painful reaction in the female when he withdraws. However, this is part of the natural process. The pain it causes triggers ovulation in the female.
