Probably drugs. I have heard that some drugs (crack, meth) makes you feel as if there are bugs under your skin. But a BUTTER KNIFE??? I would have at least asked for a steak knife. :-0

he woke up in the Denny's and had some ugly chick sleeping on his arm so rather than waking her he chopped his own arm off with all he could find... a butter knife

Maybe he had nerve damage where he constantly felt pain or insatiable itching. I've heard that happen with frostbite victims.
