Depends on you! Personal preference. How dirty are ya when you go to bed? See there are too many variances to give you a definitive answer. mr Bill

I do 3 or 4 times a year or when needed.I do it more in the summer due to having a sweaty head.

I have several extra's in the house and rotate the pillows every time I change the sheets and wash them before they go back to the bed...
