I think they make the man (or woman for that matter) take counselling first to ensure they are ready. The man has to take hormones which over a long period of time I believe changes his voice and makes him grow breasts. I believe then it is a matter of having a surgeon perform the surgery which changes his organ from male to female. I think that the hormone medication has to be taken forever.
